Health & Wellbeing Monitoring

Daily observations, clear records and timely escalation to professionals, helping you stay safe, well and independent at home.

About Health & Wellbeing Monitoring

Spotting Changes Early, keeping You Safe

Staying well at home relies on noticing small changes before they become big problems. Our carers record day-to-day observations (such as mood, appetite, hydration, mobility and skin condition), note any concerns and escalate promptly to families or health professionals when needed.

We tailor a simple monitoring plan around your needs. Where appropriate, we track basics like temperature or pulse, keep food/fluid charts, and review medication effects, always recording clearly so everyone stays informed.

Where agreed, we can note basic measures like temperature or pulse. We don’t diagnose conditions — we observe, record and escalate concerns.

We follow your agreed plan, contacting family first or liaising with the GP/district nurse/111 when appropriate. In an emergency we call 999.
